What is IHS?

Individualized Home Supports are services for people who live in their own home or family home that provide support and/or training in areas of community living such as Community Participation, Health-safety and wellness, Household management, and  Adaptive Skills.  BrightPath offers 2  types of IHS: Without training and With training.All types of IHS can be provided in the person’s own home, family’s home or in community spaces used by the general public, and either in person or remotely.

Who Qualifies for  IHS?

An individual is eligible to receive IHS  if they meet the following criteria:

             1. They live in their own home or their family’s home

      2. The individual receives services through a disability waiver.  Accepted waivers include BI, CADI, CAC, and DD.

What are BrightPath’s options for IHS?

The IHS service options available to a person are based on their age, living arrangement and the type of support and/or training they need.

Without training is provided to adults or children when they need support in at least one of the community living service categories. With training is provided to adults when they need support and training in at least one of the community living service categories.

How do the Community Living Services categories differ from each other?

Our IHS covers community living services in four categories:

Community Participation: This includes things like getting around safely, accessing community resources, building social networks, and more.

Health, Safety, and Wellness: We help you with health care coordination, self-care, health services support, and developing strategies to reach your wellness goals.

Household Management: From budgeting and meal planning to household chores and safety, we provide support for maintaining your home.

Adaptive Skills: This involves crisis prevention, positive behavior support, problem-solving, and developing skills for self-sufficiency.
