What is 24 Hour Emergency Assistance (24hr EA)?

On-call counseling, on-call problem solving and/or immediate response for assistance at a person's home due to a health or personal emergency that is not related to an injury or an immediate danger. This service does not replace the use of 911 when necessary.

 

Who Qualifies for 24hr EA?

An individual is eligible to receive 24hr EA if they meet one of the following criteria:

1. The individual served lives in an own home setting and does not receive 24-hour supervision OR the individual lives with a primary caregiver who would otherwise require extensive support in the absence of 24hr EA

2. The individual is on a waiver. Accepted waivers include BI, CADI, CAC and DD

Who Could Benefit from 24hr EA?

This service is a great support for individuals needing a 24/7 plan of care as well as individuals who are transitioning into living independently, but still need additional supports on an as-needed basis at any time. The 24-hour Emergency Assistance

provides 24/7 support including access to staff for problem solving, crisis prevention, or responding to personal emergencies. The Emergency Assistance team is one phone call away and there is always a staff that is physically able to respond at any time in the person’s home or community.

What are the different levels of 24hr EA?

Each county determines their own Tiers and Rates. However, most counties use a spectrum, where the lower level is only phone intervention, and the higher level also allows for in person supports.
